We have taken a multi-year journey in collaboration with our patients and partners across the health system to enhance and deliver transformative models of care to meet the greatest health needs of our communities. We have invested heavily in innovative technology solutions and have successfully integrated electronic medical record system which has allowed us to integrate our patients’ care between primary and secondary care. We are also heavily invested in our National Integrated Care Program with aim to make services available for our patients.
